ATTENTION ALL OPERATORS:
Effective July 1, 2004, all well logs including Five Inch (5") are required to be submitted
to the appropriate district office. Well logs should not be submitted to the Baton Rouge Office.

electric logs checked out of the Monroe Office are due
back in the Monroe Office by 4:30 P.M. the following day.
NO EXCEPTIONS! Failure to comply with this
policy will result in termination of your company's privilege
to check logs out of this office in the future.

| DM-1-R
and DT-1 Facts and Filing Information |
Listed
here are a few important facts regarding the DM-1-R and
DT-1:
- All
wells should be listed in LUW CODE ORDER
and WITHIN LUW CODE by ASCENDING SERIAL NUMBER
order - failure to do this will have all forms RETURNED
to the operator for compliance.
- All
test dates for active wells should be within six (6)
weeks prior to the due date of the report (i.e. report
date -September 1, 1999 - test date no earlier than
July 15, 1999).
- Shut-in
status should be the actual shut-in date (or last day
the well produced) [i.e. 2/1/88].
- PERFORATIONS
must match what is currently on file with our office.
If they do not match, an allowable can not be issued
or reinstated until the operator clarifies what is correct
either by correcting the DM-1-R/DT-1 or by submitting
recompletion paperwork for the new set of perforations.
- Wells
back on production after being shut-in OVER
a year will require a letter from the operator indicating
when the well went back on production and giving a current
test.
- The
font used for the reports must be legible. Small fonts
(i.e. this size and smaller are not acceptable).
It is too difficult to determine the numbers when reading
small fonts.
- Type
the name of the company agent who signs the report underneath
his/her signature along with a telephone number of that
person to contact for questions or concerns.

and DT-1 Report Dates |
Report dates for DM-1-R
and DT-1's [The first of May and November].
Oil
and Gas Proration Dates:
May 1st
DM-1-R/DT-1 is utilized to prorate allowables to be effective July
1 through December 31 [each year].
November 1st
DM-1-R/DT-1 is utilized to prorate allowables to be effective January
1 through June 30 [each year].
The
report dates underlined above are "CRITICAL
REPORT DATES". If we do not receive
these specific reports your company's allowables will
be CANCELLED. No allowable will
be reinstated until the specific form is received in the
district office and "DATE STAMPED". Your allowable
will be reinstated the date that the form is date stamped.

indicate the LUW code number according to one of the examples

For
SHUT-IN wells on DM-1-R and DT-1:
In
the "STATUS IF IN-ACTIVE" column if your well has
future utility you would indicate a "1";
if your well has NO future utility you would indicate
a "2".
In
the "DATE OF TEST OR STATUS" column you would indicate
the last date of production (or the actual last day the
well produced).

WHEN
AN OPERATOR WANTS TO TEST AND/OR PRODUCE A WELL FOR WHICH
A UNIT IS REQUIRED OR FOR WHICH A UNIT HAS BEEN APPLIED
FOR; A REQUEST FOR A TEST CLEARANCE AND/OR A 90-DAY TEST
ALLOWABLE SHALL INCLUDE THE FOLLOWING INFORMATION IN A

A statement to the effect that all monies GENERATED
from the first date of production shall be disbursed
based upon the unit to be created.

FOR TEST CLEARANCE: Date(s) of test production along
with total amount of liquids and gas produced.

FOR 90-DAY TEST ALLOWABLE: Date of first production
for a 90-Day test allowable (note: Form Completion/Recompletion
report required prior to allowable being issued)

Gas purchaser and oil transporter

Status of unitization application
NOTE:
NO EXTENSION OF THIS ALLOWABLE WILL BE GRANTED
UNLESS A UNIT HAS BEEN APPLIED FOR!

Well Report (Form WR-1) |
INACTIVE
WELLS are those in which no production operations or use as a
service well were conducted in the six-month period preceding MAY 1 and
NOVEMBER 1 of each year.
This report is due
in the district office where the wells are located, in duplicate, on the
1st day of May and the 1st day of NOVEMBER each year.
This
report is for all shut-in wells operated by a single operator.
You can list all of your SHUT-IN wells on this same report
[all fields/all wells/ONE operator].
Leave
one space between each field.
All
of the SHUT-IN wells listed below must listed on the WR-1:
UTILITY
STATUS CODE SHALL BE SHOWN AS:
- DM-1-R
and DT-1 (status if-inactive codes "1" and "2")
- All
wells which carry a status code of "31" [shut in Dry-Hole
- future utility]*
- All
wells which carry a status code of "32" [shut in Dry
Hole - no future utility]*
*The
wells that carry status code 31 or 32 usually were
shut-in after the drilling phase and have never produced.*
